• MC, DOT, or State Registration Number
• A carrier contact authorized to enter and sign the KBX Carrier Agreement
• A valid electronic W-9 (or W-8BEN-E for Canadian carriers)
• A Notice of Assignment (if using a factoring company) or electronic payment information
• A Certificate of Insurance (submitted directly by your insurance agent) that meets the minimums below
• A minimum of 20 asset trucks under the registered authority
• An NMFTA issued SCAC

• Cargo: $100,000
• Auto liability: $1,000,000
• General liability: $1,000,000 per occurrence
• Workers’ compensation: Statutory limits + Employers Liability (must check ‘Per Statute’)
• List KBX as additional insured
• Indicate the policy is primary
• Include a Waiver of Subrogation in favor of KBX
